Documenting PA Shadowing

How do I go about documenting PA shadowing? I've heard some schools have a form that can be filled out. Any info would be helpful. I'm starting in 2 weeks.

the first place to look would be on the websites of any and all schools you are interested in. all schools favor if not outright require some shadowing experience, and a few of them have their own form. so do that first. if none of the schools you are interested in have such a form, find a school that does, print it out, and have it signed at the time of shadowing IN CASE you end up needing it later, it is always easier to get such things on the spot than hunt down the person later and take up more of their time.... you could also make a generic-type form yourself that documents the pertinent info and gives the PA a place to sign..

like i said, first look at the programs yr actually interested in, sometimes these things are a little buried in their site.
